Hello! I am a single working mother with two children that have disabilities and are currently on a waiver that provides them certain services to live and work in our community. My children, an autistic young adult and a medically complex elementary school aged individual, have been working with our current service facilitator for about 8 years. Before then we had 6 other individuals who either didn’t work out, quit the profession or was extremely overwhelmed they couldn’t keep up with our families’ needs. Our current SF knows my kids, my kids know and trust our SF. This person does so much more than push paperwork. Our SF is a lifeline. When Medicaid dropped my 10 yo from coverage (100% gtube dependent and has 15 doctors/providers), our SF helped me through the process of reactivating her medical coverage. This would not happen if I had to call a service broker. This is just one example of our SF assisting us in navigating this overly complex system. Eliminating the relationship based Service Facilitator model would not just be a huge mistake but rather my family would cease to be thriving members of our community. We would likely fall through the ‘cracks’ of the system. I could potentially have to leave my job (that I love) to care for my children and become dependent on the government for survival. I would much rather continue working and providing a safe environment for my family. Our SF helps me worry less about the services my kids receive. I am humbly requesting that you reconsider the service broker model and keep the service facilitator model that has been working for our family.
