As a professional statistician I can assure you that the existing boxes serve an important purpose in assuring full understanding of the form and in preventing unreliable responses. The technique of including related or redundant responses on a form is well known in the design of effective and accurate data forms and resistance to this approach is an invitation to erroneous or misleading responses.
