Virginia Department of Medical Assistance Services changes "SUPPORT BROKER MODEL"
I am writing on behalf of my son Corey. I am his legal guardian.
My son Corey (Adult with Autism) relies on service facilitation to navigate the complex system of services and support that are provided by the State of Virginia.
Corey needs to live safely, independently, and with dignity. He currently has the right to choose his own service facilitator, and assigning a broker would take away that important choice and control over services.
Corey’s relationship with “Pathways Service Facilitation, LLC” is built on trust, understanding, and knowledge of Corey’s individual needs. This relationship provides consistency, guidance, and advocacy that cannot simply be replaced by assigning someone new. Corey's choice of provider is an essential part of his independence and self-determination. I urge you to consider the real and lasting impact this decision will have on individuals and families who depend on these services.
Please protect Corey’s right to choose a provider who knows him, understands his unique needs, and help ensure that Corey is able to continue to live the best life possible.
