Please do not revise the program by replacing Care Facilitators with Support Brokers.  The idea that we were able to choose a Care Facilitator in our area and one who meets our needs is very important.  The monthly visit is very informative, and we have developed a comfortable relationship with someone coming into our home.  Our Care Facilitator has been the only consistent piece of this ever changing puzzle.
