Guidance Document Change: This guidance document summarizes how the State Health Commissioner (“The Commissioner”) of the Virginia Department of Health (VDH), through the Office of Licensure and Certification (OLC), may consider imposing administrative sanctions for nursing facilities under 12VAC5-371-90 and Va. Code §§ 32.1-27, 27.1, and 135. This guidance document does not create new requirements or mandatory enforcement levels. Decisions regarding sanctions will always be fact specific. This guidance document does not limit VDH's or the Commissioner’s authority to take any action authorized by law.

There’s not enough words to express the disgust of this place. The rooms, medical equipment, overall facility is so outdated (except for the walk in lobby oh that’s somewhat fancy) but go through those doors and the smell of urine and neglect will knock you on your tail. Abuse, racism, neglect, I PERSONALLY witnessed it all being there all day every day for 3 months. Inspect those bedridden patients. I bet every single one has a bedsore if not bedsores from NEVER being moved. My husband sat in his own feces and urine in the geriatric chair for 7 hours. Yes, 7 hours neglected. He would cry when a certain nurse came in the room (he never cried with anyone else). Then wanted me to apply for Medicaid when we had good insurance which was in their network. Fraud is ramped in this place. VCU must be getting a fat commission for signing patients to this neglected racist facility and oh yes the “white privilege” racism runs deep. They know who they are. I wouldn’t trust those aids or nurses with a blind toothless dog much less your loved one. I would die in the street before ever having to go to ANYWHERE like this place.
