8VAC20-781-580 A.4. TOPICAL SKIN PRODUCTS
 

A.4.  "A record shall be kept that includes the child's name, the name of the product, the date, and time of use, any adverse reactions, and any application errors and action taken."

   * Such record keeping requirements ("...time of use, any adverse reactions, and any application error and action taken.")......concerning lip balm, hand lotion, sunscreen (spray and lotion), and insect repellent  - this information is not needed for children at a minimum of 16 months and older.  This is a waste of valuable time and energy. Let's give our parents the benefit of the doubt and trust they know what type of lip balm, hand lotion, sunscreen and insect repellent they are authorizing centers and staff to apply.

   * It could be argued the same should apply for diaper cream.  Parents know what kind of diaper cream they want/are using for their child and PROVIDING centers to administer.

   * While parent authorization is necessary, a clause should be added to include notification of any adverse reactions.

   * With respect to "application errors and actions taken." - no product listed above would indicate an overdose of some kind, therefore, not certain why it is necessary to include the phrase and why centers would be required to create a new form indicating such.
