A. Podiatrists' services.

1. Covered podiatry services are defined as reasonable and necessary diagnostic, medical, or surgical treatment of disease, injury, or defects of the human foot. These services must be within the scope of the license of the podiatrists' profession and defined by state law.

2. The following services are not covered: preventive health care, including routine foot care; treatment of structural misalignment not requiring surgery; cutting or removal of corns, warts, or calluses; experimental procedures; acupuncture.

3. The Program may place appropriate limits on a service based on medical necessity or for utilization control, or both.

B. Optometrists' services. Diagnostic examination and optometric treatment procedures and services by ophthalmologists, licensed optometrists, and opticians, as allowed by the Code of Virginia and by regulations of the Boards of Medicine and Optometry, acting within their scope of practice as defined under state law are covered for all recipients. Routine refractions are limited to once in 24 months except as may be authorized by the agency.

C. Chiropractors' services are not provided.

D. In accordance with 42 CFR 440.60, licensed practitioners (including an LMHP, LMHP-R, LMHP-RP, or LMHP-S (as defined in 12VAC30-50-130 or a licensed school psychologist as defined in § 54.1-3600 of the Code of Virginia) may provide medical care or any other type of remedial care or services, other than physician's services, within the scope of practice as defined by under state law. These practitioners are limited to the following:

1. Licensed Practitioners:

a. Nurse Practitioners

b. Physician assistants

c. Nurse anesthetists

d. Clinical nurse specialists

e. Respiratory therapists

f. Licensed midwives/certified professional midwives

g. Licensed clinical psychologists

h. Licensed professional counselors

i. Licensed clinical social workers

j. Licensed substance abuse treatment practitioners

k. Licensed marriage and family therapists

l. Licensed behavior analysts

m. Licensed school psychologists

2. Non-Licensed Practitioners. Residents/supervisees working under the supervision of a licensed clinical psychologist, licensed professional counselor, licensed clinical social worker, licensed substance abuse treatment practitioner or licensed marriage and family therapist in accordance with state law.

E. Addiction and recovery treatment services shall be covered in other licensed practitioner services consistent with Part XX (12VAC30-130-5000 et seq.) of 12VAC30-130.

F. Pharmacist, Pharmacy Intern and Pharmacy Technician Services. Services provided by licensed pharmacists, and pharmacy interns and pharmacy technicians supervised by licensed pharmacists, are covered when those services are provided by licensed pharmacists, and pharmacy interns and pharmacy technicians supervised by licensed pharmacists, who are acting within their scope of practice or in a collaborative agreement with a provider licensed in Virginia or are specified in Board of Pharmacy protocols for licensure that have been reviewed and accepted by DMAS and are services covered by Medicaid. Collaborative agreements can be with any licensed podiatrist or licensed advanced practice registered nurse or physician assistant. The scope of services that are covered under a collaborative agreement are limited to those under the licensed provider's scope of practice.
