Service Facilitation Transition to Support Brokers under Consumer Directed Care Network

Rationale for Service Model Transition

Where is the publicly available data to explicity outline the rationale for transitioning service facilitation into a support broker model.

Appendix H - Quality Improvement

The performance methodologies for monitoring quality improvement are outlined, but there is no documentation referenced of what, if any,  key performance indicators have not been met by the current structure and/or how those will be addressed through future measurements and/or structures.

Part I - 2024 Comments

Part I found on page 10 of 341 within 1915C, as drafted, references comments made during the 2024 public comment period expressing feedback on the service facilitator service. DMAS' written response to this feedback was that they were "disheartened" by the feedback which spoke to backlogs, interruption of services, etc..., however, there is no publicly or easily accessible data to reflect what actionable efforts DMAS made to identify and/or address the contributing factors to that experience, rather their response merely included that there is mandated service provider training required every 5 years by order of the General Assembly.

  1. How are the issues providing the service facilitation activities being mitigated by transitioning to the support broker model?
  2. What is CDCN's staffing plan recruit, train, and retain the number of employees needed to provide support within each region?
  3. What, if any, changes are being made to eliminate the degree requirements for service facilitators/support brokers?
  4. What, if any, opportunities were given to implement the structural changes allowed in the support broker model into the service facilitator model?

Consumer Directed Care Network Contract

Phase 4 Ongoing Monitoring

Language in this section indicates that based on legislative changes the state will contract with its Fiscal Employer to transition from service facilitation to support brokers...

  • If legislation is currently pending how are actions currently being taken by CDCN to prepare for the support broker activities?
    • Is this service currently written into the scope of the CDCN contract?
  • It appears that there has been no open compeition solicitation for these services...if sole sourced, where can we find the written sole source justification?
  • Where can we find the financial analysis data on the change in this service model?

Phased Implementation Outline

There is an indication that there will be a phased approach to implementation, however, that phased plan has not been shared to date.

  • DMAS states that they will provide information sessions “closer to March 1, 2027”
    • Two regions are expected to begin their transition simultaneously on March 1, 2027
  • There are no dates scheduled between transitions to reassess and respond to any identified needed changes as lessons are realized from the prior phase.
  • The final regional implementation is scheduled for June 1, 2027 but service for facilitation is scheduled to end on May 31, 2027
  • What considerations are being made for high liklihood of the loss of service facilitator personnel during this transition period, and how waiver receipients may be negatively impacted by those gaps?
