I feel that we should keep the Inerim Report but it needs a makeover.  The words Interim Report is confusing to the customers and they don't know what it means. The form needs to say something like '6  or 12 Month Recertification, Change Report, Household Update' or some other plain to the point title.  The body of the report should be changed to a question format without the grid lines. There is the problem of customers returning the report and bombarding workers on the first of the month because they don't have any benefits on their card. I think the due date on the report needs to be bigger or stand out in some way.
