Amend regulations to conform to legislation enacted by 2014 General Assembly

| Action Summary |
The amendments make the following changes:
Remove the requirement for a booster dose of diphtheria or tetanus-containing vaccine at 6th grade entry.
Remove the 5-year interval language from the requirement for a booster dose of pertussis-containing vaccine at the 6th grade entry.
Remove the word “susceptible” from the varicella vaccine requirement.
Modify the pneumococcal vaccine requirement to include children up to 60 months of age.
