| Purpose of the meeting Commonwealth of Virginia, Virginia Health Department (VDH), Notice of Intent to hold a public hearing and advisory committee meeting for the Preventive Health and Health Service (PHHS) Block Grant. All interested individuals and groups are invited to participate at the public hearing on the PHHS Block Grant. In accordance with Title XIX, Section 1905 of the Public Health Service Act, the Commonwealth of Virginia hereby gives notice that VDH will apply for FY 2018 PHHS Block Grant funds for programs addressing Healthy People 2020 national health objectives. The public hearing will be held on Monday, July 8, 2019 from 10:00 – 12:00 p.m. during the advisory committee meeting. The public hearing and advisory committee meeting will be held at the Virginia Perimeter Center; Conference Center; Hearing Rm 3; 9960 Mayland Drive, Henrico, Virginia 23233. The State Plan will be available on the agency’s website at www.vdh.virginia.gov/vdhlivewell/. Public comment on the plan can be made at the public hearing and written comments can be addressed to Heather Board, Office of Family Health Services, P.O. Box 2448, 9th Floor, Richmond, VA 23219. | |
