It seems to me, common sense demands leaving the citizen and felon check boxes on the form. Upon seeing these boxes, the prospective voter has an opportunituy to make a decision regardiong his or her qualifications to be a voter, and whether or not to accept the consequences of falsely claiming to qualify. Embedding the requirements in a certification which most people don't read is not a suitable alternative. Moreover, I think most prospective voters would be proud to check these two boxes.
