The SBE should be concentrating on finding ways to ensure fair elections.  Changing the form is not the problem, the problem is making sure that poll workers follow the rules, the machines record the vote properly, and ensure that the person voting has identification.  The current registration form is simple and to the point. It works, so don't spend tax dollars to fix something that isn't broken.
