It is inconsistent to have check boxes that are identified as mandatory on the form and then state in the regulations that the empty boxes can be ignored. The boxes have a purpose -- to ensure that the person acknowledges and fulfills the requirements. To make a determination that will be public will send a contradictory message as to whether the requirements are truly required.
Additionally, if a person registers on-line, they are required to fill in all of the boxes before continuing on. The paper form requirements should be as stringent as the web-based registrations.
The middle name changes should also be dropped. In Fairfax County the use of the middle name to identify one of 700,000 voters is critical. The logic that the register would call the person and only reject the application if that talk to the person and find to they do in fact have a middle name yet accept the application if the person does not answer the phone. The default should be to reject as opposed to accept if the middle name is blank.
