Voting is a right -- for those who are qualified to vote, and a privelege. If a "potential" voter is serious about claiming his/her right and availing of the privelege, then it is up to that person to make sure that he/she fills out that form (1) truthfully and (2) completely. The only thing that I might suggest is to  make it a standard procedure to return incomplete forms  to the "applicant" AND require them to re- submit their completed forms at DMV where they should be required re-present identification documents. If you make it just as tedious to re-submit, then may be they will do it right the first time. Maybe.
