Just leave the form alone. Everyone is use to it this way. It works. Why must bureaucrats make changes to justify their own jobs? Save the state money by not having to pay someone to create, print, deliver etc. new forms. And knowing the way governments work there is probably money going to be spent "training" everyone in how to use the new form. Stop already. Leave it and the citizens alone. Why not use this money to investigate voter fraud rather than creating more opportunity for fraud.
