NO! NO! NO!  " PLEASE" vote NO on this new application. Besides being way to large for our card files and the blue background will not copy well when applications are faxed to other localities. The changes recommended would open the door for all kinds of voter fraud. The current application works just fine.  You cannot assume potential voters will read the signature oath and comprehend what it says, therefore many serious problems with this form will arise.
